If you do not have any government documents, a medical certificate issued by a civil surgeon and an affidavit from a competent court can be used.

  • Log In: Enter your UAN, password, and captcha. Click on Sign-in.
  • Modify Details: Click on Manage and then select Modify Basic Details.
  • Enter Correct Information: According to your Aadhaar, enter the correct name and date of birth. Click on Save/Submit.
  • Notify Employer: Inform your employer to accept your name change request.

  • Required Proofs

    Major changes to the name, date of birth, nominee, address, or father's/husband's name in the account require paper proofs from both the employer and the shareholders. Changes to the date of birth as per Aadhaar or e-Aadhaar are accepted only up to a gap of three years from the date of birth previously recorded in the EPFO account.
